In order to register for any training you must fill out the following registration form, submit the appropriate documentation, and select what form of payment you would like to use.

If you are an Intern or Student you must have your supervisor fill out the following form and submit it before attending the training.

    If you would like to pay via PayPal go to the "Store" page of our website to pay. Checks and Payment Plan installments must be mailed to 1615 E. Warner Rd., Suite #2, Tempe, AZ, 85284
  • If you would like to request a payment plan, you must fill out the payment plan form above and submit it here.
  • If you are wanting to register for the refresher course, you must submit documentation of completion of your original basic training.
